Both parents must appear in person at a court house or post office and present
the original documents to an acceptance agent which is authorized to accept passport
applications. If both parents are not available, please contact our office at
(310) 909-7526. The acceptance agent is needed to witness the signing of your
application for the passport.
Notify the agent you are using an expediting service to "hand carry" your
application. Your child must go along to the post office or court house. If the child is 16 or older, only one parent is required. Most facilities are open M-F, 9am to 4pm.
1. To verify your identity, the agent will request your proof of identity.
2. The agent will ask you to sign the DS-11.
3. To verify that you need an expedited passport, you will need to show them
your child's travel documentation.
4. The agent will then seal the DS-11,
photos, proof of citizenship, travel documents, authorization letter and the $120.00 check
in an envelope marked "To
be Opened by U.S. Passport Office Only".
5. Important: Make sure the agent returns the sealed envelope to you. If the
agent does not feel at ease about releasing the envelope, suggest that he/she
refer to page 31 of their Passport Agent's Manual, "Hand-carrying of Executed
Applications".
DO NOT OPEN THIS SEALED ENVELOPE OR THE APPLICATION
WILL BE INVALID.
