1) COMPLETE THE APPLICATION FORM DS-11
You must fill out the online application and print it out. Once the application is completed you must have it witnessed by going to a Passport Acceptance Facility.


DO NOT SIGN THIS DOCUMENT UNTIL INSTRUCTED DO SO IN STEP 8.

2) PROOF OF U.S. CITIZENSHIP ( minors under 16)

For Minors under Age 16 you must establish proof of citizenship AND proof of relationship.  Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).  See number 3 below for proof of relationship.

You must provide an official copy of ONE of the following documents.

If your were born in the United States:

»Original Birth Certificate issued by City, County or State
» Previous U.S. Passport

3) PROOF OF IDENTITY

You must provide an official copy of ONE of the following documents as proof of Identity:

  1. Both parent's current, valid Driver's License with Photo (must be issued greater than 6 months ago)
  2. If the above mentioned Identification isn't available, then you need to supply an Alternate Proof of Identity
You must provide an official copy of ONE of the following documents as proof of Relationship:

  1. Certified U.S. birth certificate (with parents' names); or
  2. Certified Foreign Birth Certificate (with parents' names and translation, if necessary); or
  3. Report of Birth Abroad (Form FS-240) (with parents' names); or
  4. Certification of Birth Abroad ( Form DS-1350) (with parents' names); or
  5. Adoption Decree ( with adopting parents' names); or
  6. Court Order Establishing Custody; or
  7. Court Order Establishing Guardianship.

NOTE: If the parent(s)'/guardian's name(s) is/are other than that on these documents, evidence of legal name change is required.


4) PASSPORT PHOTOGRAPHS
You must provide two identical passport photos that meet Passport Photograph Regulations.

5) LETTER OF AUTHORIZATION

»Print and complete the Parental Application Permission Documentation .

6) PROOF OF DEPARTURE

You must provide a copy of ONE of the following:

» Your foreign travel plane ticket
» Your printed travel itinerary showing foreign travel
» If this is a business trip, you may present a business letter from your employer



7) FEES AND CUSTOMER INFORMATION FORM

Government Fee

Make one check payable to "US Department of State" for $120.00
(this is the government processing fee)

Be sure to include your date of birth on the check
» Starter Checks and 3rd-Party Checks will NOT be accepted.
» This check will go into a sealed, government envelope with documents mentioned above.


Make a second check or money order in the amount of $25.00 payable to the Post Office or Court House. (*The Acceptance Agent)

8) AUTHORIZATION
Call us at (310) 909-7526 to indicate that you're processing an Emergency Passport from the Internet.

Prior to authorization, verify that you have collected the following documents:

1. Application Form DS-11 (both pages, if downloaded)
2. Proof of Citizenship
3. Proof of Identity
4. Two Passport Photographs
5. Parental Application Permission Documentation (if necessary) And Authorization Letter
6. Travel Documentation
7. Government Fees


Both parents must appear in person at a court house or post office and present the original documents to an acceptance agent which is authorized to accept passport applications. If both parents are not available, please contact our office at (310) 909-7526. The acceptance agent is needed to witness the signing of your application for the passport.

Notify the agent you are using an expediting service to "hand carry" your application. Your child must go along to the post office or court house. If the child is 16 or older, only one parent is required. Most facilities are open M-F, 9am to 4pm.

The acceptance agent will take about 10 minutes to explain the following steps:

1. To verify your identity, the agent will request your proof of identity.
2. The agent will ask you to sign the DS-11.
3. To verify that you need an expedited passport, you will need to show them your child's travel documentation.
4. The agent will then seal the DS-11, photos, proof of citizenship, travel documents, authorization letter and the $120.00 check in an envelope marked "To be Opened by U.S. Passport Office Only".
5. Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".

DO NOT OPEN THIS SEALED ENVELOPE OR THE APPLICATION WILL BE INVALID.
